How to complete fillable PDF forms

To open and complete the attached PDF form, you will need Adobe Reader (the latest

version is recommended). If it is not installed on your computer, you may

download the latest version free of charge from https://get.adobe.com/reader/ .

 

For Otis staff: feel free to contact the helpdesk [email protected] if you are asked for administrator username and password when attempting installation.

 

  1. Download and save the form to your computer before attempting to complete it ( Adobe Acrobat Reader must be installed on your computer ).
  2. Use Adobe Acrobat Reader to open the original PDF form that you saved on your own computer .
  3. Complete the form .
  4. From the Reader menu bar, select  [File] > [Save As] .
  5. Select or type a name that is different from the original.
  6. Under Format, chose [Adobe PDF Files] .
  7. Choose a location to save the file, then click [Save] .
  8. Now you can attach the completed .pdf file to the email you wish to send.

 

NOTE: Mac Users: do not use Preview to fill forms

For Macintosh system users: DO NOT use the Preview program to fill in the PDF form.

 

Adobe Reader is the only program that will allow you to work with .pdf forms, and allow your recipient to view or print the completed form properly. If you do not already have Adobe Reader, please use the link above to install it to your machine.
