Who Completes the Form
- The form can be filled out by either the employee or their manager/supervisor.
Approval Workflow
- Initial Approval:
Approval from the department manager or supervisor. - Review by Data Custodians:
The form is reviewed and approved by the Banner Data Custodians. - Final IT Approval:
IT provides final approval before proceeding.
Automated Ticket Creation
Once the form receives all necessary approvals, a ticket is automatically generated in the HappyFox IT Ticketing System to initiate the access provisioning process.
